A 36-year-old patient came to Ingenes hoping for a second baby. Her first child had been born after IVF in 2020, but this time the picture was far more complex: Polyendocrine Metabolic Ovarian Syndrome (PMOS, formerly Polycystic Ovary Syndrome or PCOS), excess weight, insulin resistance with hyperinsulinemia, and male-factor infertility from teratozoospermia in her partner. In the first cycle, only one of four embryos was chromosomally normal, her endometrium measured just 3.6 mm (well below the 7 mm minimum), and the transfer did not implant.
Rather than repeating the same protocol, the Ingenes team addressed every point of failure at once, with a four-level protocol that is the essence of TripleMed™:
- Level 1 · Metabolic
A ketosis-based nutritional intervention to correct insulin resistance. Within two weeks, her HOMA-IR index fell below 2.0.
- Level 2 · Regenerative and Genetic
The Implantation Score, a molecular assessment of egg quality developed by the Ingenes research team. Three eggs scored above the viability threshold.
- Level 3 · Genetic
PGT-A to confirm chromosomal normality. Three of five embryos were euploid.
- Level 4 · Regenerative
Endogen™ during endometrial preparation. The endometrium grew from 3.6 mm to an ideal 11.8 mm trilaminar lining.
